What is Spirometry?
Spirometry is a simple, non-invasive breathing test that measures how much air your lungs can hold and how fast you can breathe in and out. It is performed using a device called a spirometer, which records your breathing effort as a graph called a spirogram.
The test is recommended by the American Thoracic Society (ATS) and European Respiratory Society (ERS) as the primary diagnostic tool for obstructive and restrictive lung diseases.

What Spirometry Measures
Four key values that together reveal the health of your airways and lung capacity.
Total air you can forcefully exhale after a deep breath. Reduced FVC may indicate restrictive lung disease.
Air exhaled in the first second of forced breath. Key marker for airway obstruction in Asthma and COPD.
The most important value. Below 0.70 confirms obstructive disease. Normal with low FVC suggests restriction.
Maximum speed of exhalation. Used to monitor Asthma severity and response to bronchodilators.

How the Test is Performed

Sit comfortably
You sit upright in a chair. A nose clip is placed to ensure all breathing goes through the mouthpiece.
Breathe normally
You breathe normally for a few seconds so the machine establishes your baseline breathing pattern.
Deep breath in
You inhale as deeply as possible, filling your lungs completely before the forced manoeuvre.
Blow out hard & fast
You exhale as forcefully and completely as possible into the mouthpiece for at least 6 seconds.
Repeat 3 times
The manoeuvre is repeated 3 times to ensure reproducible, accurate results per ATS/ERS guidelines.

Is spirometry painful or risky?
Spirometry is completely painless and non-invasive — no needle, no radiation, no discomfort. Some patients feel briefly lightheaded after forced exhalation; this is normal and passes within seconds.
How should I prepare for the test?
Avoid smoking for 4 hours before. Do not use short-acting inhalers (bronchodilators) for 4–6 hours before, unless instructed otherwise. Eat a light meal and wear comfortable, loose clothing.
What do the results mean?
FEV₁/FVC below 0.70 confirms obstructive disease (Asthma, COPD). Reduced FVC with a normal ratio suggests restrictive disease (ILD, fibrosis). Dr Nilesh Sonawane explains results in detail on the same visit.
How long does the test take?
The test itself takes 15–20 minutes. Results are reviewed immediately and the full consultation including diagnosis and treatment plan is completed in the same visit.
Who needs a spirometry test?
